AI Assistant (in Pabbly Chatflow) keeps replying Fallback Message only

The AI Assistant is setup with knowledge base (as attached PDF file).
The flow is setup.
My AI Assistant keeps replying Fallback Message only, I have no idea which part is wrong. Please assist.
