Sheets Lookup Row not showing separate fields in response

Google Sheets Lookup Row is no longer expanding the response into the separate columns of data from Google Sheets. I'm after the row index but it is combined with all the other info in one box.

It appears this may be a cache-related issue. Please clear your browser cache, select Simple Response, and then click Save and Send Test Request button.
